Free Blood Glucose Meters for County Health Plan Members

Blood glucose meters are available at no cost to Plan A and B members with diabetes.

The Bayer's BREEZE2 and CONTOUR meters, test strips and disks are the only blood glucose meters, test strips and disks covered under the County Health Plan's pharmacy benefit.

Members may choose either the Bayer BREEZE2, which uses a disk, or the Bayer CONTOUR meter, which uses test strips.  Both of these meters offer No Coding technology for accurate results.

Members with diabetes should be encouraged to call Bayer Customer Service at 1-877-229-3777 to discuss which meter would be best for them and to arrange for home delivery at no cost.  The member's County Health Plan ID number is required.
